ABSTRACT

Background Intestinal duplications are rare congenital developmental anomalies with an incidence of 0.005-0.025% of births. They are usually identified before 2 years of age and commonly affect the foregut or mid-/hindgut. However, it is very uncommon for these anomalies, to arise in the colon or present during adulthood. Case presentation Herein, we present a case of a 28-year-old woman with a long-standing history of constipation, tenesmus, and rectal prolapse. Colonoscopy results were normal. An abdominal computed tomography (CT) revealed a diffusely mildly dilated redundant colon, which was prominently stool-filled. The gastrografin enema showed ahaustral mucosal appearance of the sigmoid and descending colon with findings suggestive of tricompartmental pelvic floor prolapse, moderate-size anterior rectocele, and grade 2 sigmoidocele. A laparoscopic exploration was performed, revealing a tubular duplicated colon at the sigmoid level. A sigmoid resection rectopexy was performed. Pathologic examination supported the diagnosis. At 1-month follow-up, the patient was doing well without constipation or rectal prolapse. Conclusions Tubular colonic duplications are very rare in adults but should be considered in the differential diagnosis of chronic constipation refractory to medical therapy. Due to the non-specific manifestations of this entity, it is rather challenging to make an accurate diagnosis pre-operatively. Surgery remains the mainstay of treatment. Some reports suggest that carcinomas are more prone to develop in colonic/rectal duplications than in other GI tract duplications.

ABSTRACT

ABSTRACT Pelvic floor rehabilitation aims to address perineal functional and anatomic alterations as well as thoraco-abdominal mechanic dysfunctions leading to procto-urologic diseases like constipation, fecal and urinary incontinence, and pelvic pain. They require a multidimensional approach, with a significant impact on patients quality of life. An exhaustive clinical and instrumental protocol to assess defecation disorders should include clinical and instrumental evaluation as well as several clinical/physiatric parameters. All these parameters must be considered in order to recognize and define any potential factor playing a role in the functional aspects of incontinence, constipation and pelvic pain. After such evaluation, having precisely identified any thoraco-abdomino-perineal anatomic and functional alterations, a pelvi-perineal rehabilitation program can be carried out to correct the abovementioned alterations and to obtain clinical improvement. The success of the rehabilitative process is linked to several factors such as a careful evaluation of the patient, aimed to select the most appropriate and specific targeted rehabilitative therapy, the therapist's scrupulous hard work, especially as regards the patient's emotional and psychic state, and finally the patient's compliance in undertaking the therapy itself, especially at home. These factors may deeply influence the overall outcomes of the rehabilitative therapies, ranging from "real" success to illusion "myth".


RESUMO A reabilitação do assoalho pélvico visa abordar alterações funcionais e anatômicas perineais, bem como disfunções mecânicas torácicas-abdominais que levam a doenças procto-urológicas como prisão de ventre, incontinência fecal e urinária e dor pélvica. Requerem uma abordagem multidimensional, com impacto significativo na qualidade de vida dos pacientes. Um protocolo clínico e instrumental exaustivo para avaliar os transtornos de defecação deve incluir avaliação clínica e instrumental, bem como diversos parâmetros clínicos/fisiátricos. Todos esses parâmetros devem ser considerados para reconhecer e definir qualquer fator potencial desempenhando um papel nos aspectos funcionais da incontinência, prisão de ventre e dor pélvica. Após tal avaliação, tendo identificado com precisão quaisquer alterações anatômicas e funcionais tóraco-abdomino-perineais, um programa de reabilitação pelvi-perineal pode ser realizado para corrigir as alterações acima mencionadas e obter melhora clínica. O sucesso do processo de reabilitação está ligado a diversos fatores, como uma avaliação cuidadosa do paciente, visando selecionar a terapia de reabilitação direcionada mais adequada e específica, além do trabalho árduo e escrupuloso do terapeuta, especialmente no que diz respeito ao estado emocional e psíquico do paciente e, finalmente, a conformidade do paciente em realizar a terapia em si, especialmente em casa. Esses fatores podem influenciar profundamente os resultados globais das terapias de reabilitação, que vão desde o sucesso "real" até o "mito" ilusório.

Resumen Introducción: Los reportes extranjeros dan cuenta de la presencia de esta patología en cifras que varían entre 10 y 70%, con sólo un reporte nacional centrado en ancianos institucionalizados. Existen diferentes formas de evaluar la presencia de constipación; en nuestra experiencia utilizamos los nuevos criterios de ROMA IV que define como constipación funcional cuando se cumplen dos o más criterios. Objetivos: Determinar la prevalencia y el perfil epidemiológico de pacientes adultos sanos con constipación. Material y Método: Estudio de corte transversal realizado entre enero y marzo de 2018. Se incluyen mayores de 18 años que acceden a completar la encuesta. Se excluye aquellas personas con antecedentes quirúrgicos o mórbidos gastrointestinales. Se realiza encuesta aplicando los criterios de ROMA IV considerando aspectos demográficos. Se utiliza estadística descriptiva. Resultados: De 1.500 encuestas realizadas se seleccionan 1.223. Un 68,4% cumplían con dos o más criterios de constipación funcional, con diferencias estadísticamente significativas en el grupo de sexo femenino y en pacientes de mayor edad. El análisis específico de cada criterio de ROMA IV muestra que los síntomas más frecuentes fueron deposiciones duras, esfuerzo defecatorio excesivo y sensación de evacuación incompleta. Hubo diferencias estadísticamente significativas en mujeres en los ítems: menor frecuencia, deposiciones duras y esfuerzo defecatorio mientras que la mayor edad se asocia a evacuación incompleta. Las maniobras digitales fueron poco frecuentes. Conclusiones: La constipación es frecuente en la población adulta sana afectando, principalmente, a mujeres de mayor edad. Los síntomas más frecuentes fueron deposiciones duras, esfuerzo defecatorio y sensación de evacuación incompleta.


Introduction: The prevalence of this condition has been reported in foreign studies in around 10-70%, with only one national report focused in the institutionalized elderly. There are different diagnostic approaches to determine the presence of functional constipation, in our experience, we use the ROME IV diagnostic criteria, which requires the presence of two or more criteria for confirmation. Aim: To determine the prevalence and epidemiologic profile of healthy adults with constipation disorder. Materials and Method: Transversal cohort study between January-March 2018. Inclusion criteria are 18 years and above. We exclude patients with gastrointestinal medical or surgical comorbidities. Data are collected from a face-to-face survey based on the ROME IV criteria, considering demographic features. Descriptive statistics are used to analyze the results. Results: From a total of 1,500 questionnaire surveys, 1,223 are selected. Two or more criteria for functional constipation were present in 68.4%, with significant statistical differences in the female and elderly population. When analyzing each criterion separately, hard stools, straining and sensation of incomplete evacuation were the most common responses. In women, less frequency, hard stools and straining items showed significant statistical differences, whereas sensation of incomplete evacuation was the highlight in the elderly. Manual maneuvers to facilitate defecation was the less selected item. Conclusions: Functional constipation is a frequent condition seen in healthy adults. Older female population are particularly prone to it. Hard stools, straining, and incomplete evacuation sensation were the most common symptoms.

Introduction and aim: Functional abdominal pain (FAP) is one of the major gastrointestinal complaints in childhood. Studies have reported occult constipation (OC) as one of the leading causes of abdominal pain. Recent researches have proposed laxatives as potent therapeutic targets for abdominal pain in patients with OC. However, no study has compared effect of poly ethylene glycol (PEG) and lactulose on occult constipation. Materials and methods: 51 patients aged 4 to 18 years with abdominal pain who had OC (defined as fecal impaction in abdominal X ray) were studied. Demographic and clinical data including age, sex, body weight, height, abdominal pain duration, abdominal pain rate and fecal odor were registered. They were randomly assigned to receive PEG (1gr/kg) or Lactulose (1cc/kg) for at least two weeks. All patients were reevaluated by pain measurement scale after at least two weeks of treatment. Results: It is indicated that the efficacy of PEG for reducing abdominal pain in OC was 48% while it was 37% for Lactulose. This study indicated that this efficacy is not affected significantly by sex and fecal odor, however this efficacy is influenced by age, body weight, abdominal pain duration and abdominal pain rate for both PEG and Lactulose. Conclusion: It could be concluded that PEG is a more efficient drug for treating abdominal pain in occult constipation than Lactulose and its optimum effect can be achieved in elder patients with more severe abdominal pain.


Introducción y objetivo: El dolor abdominal funcional (FAP) es una de las principales molestias gastrointestinales en la infancia. Los estudios han informado que el estreñimiento oculto (OC) es una de las principales causas de dolor abdominal. Investigaciones recientes han propuesto laxantes como objetivos terapéuticos potentes para el dolor abdominal en pacientes con OC. Sin embargo, ningún estudio ha comparado el efecto del polietilenglicol (PEG) y la lactulosa sobre el estreñimiento oculto. Materiales y métodos: Se estudiaron 51 pacientes de 4 a 18 años con dolor abdominal que tenían OC (definida como impactación fecal en rayos X abdominales). Se registraron datos demográficos y clínicos que incluyen edad, sexo, peso corporal, altura, duración del dolor abdominal, tasa de dolor abdominal y olor fecal. Fueron asignados aleatoriamente para recibir PEG (1 gr/kg) o lactulosa (1 cc/kg) durante al menos dos semanas. Todos los pacientes fueron reevaluados por la escala de medición del dolor después de al menos dos semanas de tratamiento. Resultados: Se indica que la eficacia de PEG para reducir el dolor abdominal en OC fue del 48% mientras que fue del 37% para la lactulosa. Este estudio indicó que esta eficacia no se ve afectada significativamente por el sexo y el olor fecal, sin embargo, esta eficacia está influenciada por la edad, el peso corporal, la duración del dolor abdominal y la tasa de dolor abdominal tanto para PEG como para lactulosa. Conclusión: Se podría concluir que el PEG es un fármaco más eficaz para tratar el dolor abdominal en el estreñimiento oculto que la lactulosa y que su efecto óptimo se puede lograr en pacientes mayores con dolor abdominal más severo.Palabras clave: dolor abdominal, estreñimiento oculto, polietilenglicol, lactulosa.

Objetivos: Describir los resultados de las manometrías anorrectales (MAR) en pacientes pediátricos con estreñimiento crónico y patología anorrectal adquirida. Materiales y métodos: Se revisaron los expedientes de pacientes pediátricos referidos entre 2004 y 2016 al Laboratorio de Motilidad Gastrointestinal del Hospital San José Tec de Monterrey para evaluación por manometría anorrectal y que presentaron patología anorrectal adquirida. Resultados: Se revisaron 170 expedientes. Edad 7,18 ± 4,51 años. La prevalencia de patología anorrectal (PA) fue de 73%. Síntomas con mayor incidencia: dificultad para evacuar (78%), dolor al evacuar (67%), heces duras (50%) e incontinencia fecal asociado (49%). El 44% de los pacientes con esfínter anal externo (EAE) hipotónico presentaron incontinencia y 74% estos últimos, presentaron menor volumen máximo tolerable (VMT). Los valores manométricos con mayor significancia: presión en reposo del EAE (promedio ± DE) 14,16 ± 10,19 en PA y de 26,08 ± 13,65 en SPA; presión en contracción del EAE 48,4 ± 34,1 en PA y 68,3 ± 37,7 en SPA; VMT 120,8 ± 60,4 en PA y de 173,2 ± 78,0 en SPA. El 97,97% de los pacientes en los que se evaluó la coordinación abdomino-pélvica tuvieron disinergia del piso pélvico. Conclusiones: A diferencia de la población adulta, los valores manométricos de niños con patología anorrectal se encontraron dentro de rangos normales excepto por el EAE y el VMT los cuales estuvieron disminuidos. Esto puede sugerir un mecanismo diferente en la población pediátrica. La disinergia del piso pélvico podría explicar el estreñimiento crónico en estos pacientes.


Objective: To describe the anorectal manometry results in the pediatric population with chronic constipation and acquired anorectal disease. Materials and methods: We reviewed the records of children who were referred to the Motility and Pelvic Floor Laboratory of the Hospital San Jose Tecnologico de Monterrey between 2004-2016 for further evaluation with anorectal manometry and who presented acquired anorectal disease. Results: We reviewed 170 records. The mean age was 7.18 ± 4.51 years old. The prevalence of anorectal disease was 73%. The symptoms more frequently presented were difficult evacuation (78%), painful defecation (67%), large and hard stool (50%) and fecal soiling (49%). 44% of patients with hypotonic external anal sphincter (EAS) presented with soiling and 74% of those had diminished critical volume. Significant manometric values (p<0.05) were EAS resting pressure, maximal squeeze pressure, and critical volume. 97.7% of those who underwent abdomino pelvic coordination evaluation had pelvic floor dyssynergia (anismus). Conclusions: Contrary to adult population, the manometric values in children with acquire anorectal pathology were within normal values except for the EAS resting pressure and critical volume that were diminished. This could suggest a different mechanism in the pediatric population. Pelvic floor dyssynergia could explain chronic constipation in these patients.

ABSTRACT Tension-type headaches (TTH) and irritable bowel syndrome (IBS) are comorbid diseases affecting, especially, women. One of the underlying mechanisms for both is autonomic dysfunction in the brain-gut axis. We aimed to evaluate the factors accompanying TTH and their relation to functional gastrointestinal disorders. Methods: Women diagnosed with TTH were questioned about headache pattern and severity, and accompanying factors, as well as being screened for IBS according to the Rome III criteria. The participants were divided into two groups: constipation-dominant IBS (IBS-C) group and "others", with a total of 115 individuals included in the study. Results: Of the 115 patients; 48 (41.8%) of the women had IBS-C criteria while 67 (58.2%) described mixed-type IBS or diarrhea-dominant IBS. There were no significant differences in terms of mean age (p = 0.290), body mass index (p = 0.212), visual analog scale (p = 0.965), duration of attacks (p = 0.692), and episodic/chronic type (p = 0.553). Osmophobia was seen in 43.5%; phonophobia in 68.7%, and photophobia in 47.0% of the patients, and only osmophobia was significantly associated in women in the IBS-C group (p = 0.001). Conclusion: In female patients with TTH, a higher level of constipation was detected. The relationship of these two diseases suggests that they may share common mechanisms. This is the first study showing the relationship of osmophobia with constipation.


RESUMO As cefaléias do tipo tensional (TTH) e a síndrome do intestino irritável (IBS) são comorbidades que afetam especialmente as mulheres. O mecanismo subjacente para ambas é a disfunção autonômica no eixo cérebro-intestino. Nosso objetivo foi avaliar os fatores que acompanham as TTH e sua relação com distúrbios gastrointestinais funcionais (FGID). Métodos: Mulheres com diagnóstico de TTH foram questionadas quanto ao padrão e gravidade da cefaleia, fatores acompanhantes e triadas para IBS de acordo com os critérios de Roma III e foram divididas em dois grupos: grupo com IBS com predominância de constipação (IBS-C) e as demais. Um total de 115 mulheres foram incluídas no estudo. Resultados: Do total de 115 pacientes; 48 (41,8%) das mulheres tiveram os critérios da IBS-C, enquanto 67 (58,2%) tiveram IBS do tipo misto ou IBS com predominância de diarreia. Não houve diferenças significativas em termos de idade média (p = 0,290), índice de massa corporal (IMC) (p = 0,212), escala visual analógica (VAS) (p = 0,965), duração dos ataques (p = 0,692), e tipo episódico / crônico (p = 0,553). Osmofobia foi observada em 43,5% das pacientes; fonofobia em 68,7% e fotofobia em 47,0%, e apenas osmofobia foi significativamente associada no grupo de mulheres com predominância de constipação (p = 0,001). Conclusão: Em pacientes do sexo feminino com TTH, encontra-se uma maior constipação. A relação dessas duas doenças sugere que elas podem compartilhar mecanismos comuns. Além disso, este é o primeiro estudo mostrando a relação da osmofobia com a constipação.

ABSTRACT BACKGROUND: An association between urinary disorders and functional constipation has been registered in children and adults, with functional constipation being a common complaint in individuals with overactive bladder. OBJECTIVE: To evaluate the prevalence of functional constipation, overactive bladder and its dry/wet subtypes in women and to determine which bowel symptoms predict overactive bladder. METHODS: A cross-sectional study of women randomly approached in public spaces. Exclusion criteria: neurological/anatomical abnormalities of the bowel or urinary tract. Constipation was defined as ≥2 positive symptoms of those listed in the Rome criteria. Urinary abnormalities (frequent urination, urgency, incontinence, nocturia) were defined by a score ≥2 in the respective item of the International Consultation on Incontinence Questionnaire - Overactive Bladder. Dry overactive bladder was defined as urgency without incontinence, while wet overactive bladder included incontinence. RESULTS: A total of 516 women with a mean age of 35.8±6 years were interviewed. Rates of functional constipation, overactive bladder, dry overactive bladder and wet overactive bladder were 34.1%, 15.3%, 8.9% and 6.4%, respectively. Functional constipation was associated with overactive bladder and dry overactive bladder, with functional constipation predicting dry overactive bladder (OR=2.47). Quality of life was poorer in constipated women compared to non-constipated and even worse in constipated women with wet overactive bladder (median 22.5; 95%CI: 17.25-35.25). Manual maneuvers were significantly associated with both overactive bladder subtypes. Independent predictive factors for overactive bladder were manual maneuvers (OR=2.21) and <3 defecations/week (OR=2.18), with the latter being the only predictive factor for dry overactive bladder (OR=3.0). CONCLUSION: Functional constipation is associated with overactive bladder and its dry subtype, particularly in the younger population. In addition, this association is responsible for lower quality of life scores, especially when urinary incontinence is present. The presence of manual maneuvers and less than three defecations per week should direct us to look for overactive bladder.


RESUMO CONTEXTO: A associação entre distúrbios urinários e constipação funcional vem sendo observada em crianças e adultos, sendo a constipação funcional uma queixa comum em indivíduos com bexiga hiperativa. OBJETIVO: Avaliar a prevalência de constipação funcional, bexiga hiperativa e seus subtipos seco/úmido em mulheres e determinar quais os sintomas intestinais estão mais associados e são preditores de bexiga hiperativa. MÉTODOS: Estudo de corte transversal com mulheres abordadas aleatoriamente em locais públicos. Os critérios de exclusão foram: anormalidades neurológicas/anatômicas do intestino ou do trato urinário documentadas. A constipação foi definida como ≥2 sintomas positivos daqueles listados nos critérios de Roma. Alterações urinárias (frequência urinária aumentada, urgência, incontinência e noctúria) foram definidas por um escore ≥2 no respectivos itens do Questionário Internacional de Consulta sobre Incontinência - Bexiga Hiperativa. Foi denominada de bexiga hiperativa seca a presença de sintomas de urgência sem incontinência urinária e bexiga hiperativa úmida quando a urgência estava associada a incontinência urinária. RESULTADOS: Foram entrevistadas 516 mulheres com idade média de 35,8±6 anos. As taxas de constipação funcional, bexiga hiperativa, bexiga hiperativa seca e bexiga hiperativa úmida na amostra estudada foram de 34,1%, 15,3%, 8,9% e 6,4%, respectivamente. Foi observada associação entre constipação funcional e bexiga hiperativa / bexiga hiperativa seca, sendo a constipação funcional fator preditor para esse subtipo de bexiga hiperativa (OR=2,47). O escore de qualidade de vida foi pior nas mulheres com constipação funcional em comparação com as não constipadas e ainda pior nas mulheres com constipação funcional associada a bexiga hiperativa úmida (mediana 22,5; IC 95%: 17,25-35,25). A presença de manobras manuais estava significativamente associada aos dois subtipos de bexiga hiperativa. Os fatores preditivos independentes para bexiga hiperativa foram manobras manuais (OR=2,21) e <3 defecações/semana (OR=2,18), sendo este último o único fator preditivo para bexiga hiperativa seca (OR=3,0). CONCLUSÃO: Em mulheres, a constipação funcional está associada a bexiga hiperativa e seu subtipo seco, particularmente na população mais jovem. Além disso, essa associação é responsável por piores escores de qualidade de vida, principalmente quando a incontinência urinária está presente. A presença de manobras manuais e menos de três defecações por semana em mulheres devem nos direcionar a procurar por bexiga hiperativa.

ABSTRACT Objective: Transanal repair of rectocele and full rectal mucosectomy with one circular stapler is a procedure designed for the treatment of Obstructive Defecation Syndrome by doctor Fco. Sergio Regadas in 2005. We compare the use of multiple instruments and their mechanical technology effect in the treatment of anorectocele. Patients and methods: Female patients complaining about sensation of incomplete evacuation, ages between 40 and 55. The evaluation was made with the function of evacuation protocol: colonic transit time, colon radiology, ecodefecography, anorectal manometry and psychological test. The technique used was transanal repair of rectocele and full rectal mucosectomy with one circular stapler, using staplers CPH-34, CPH-34HV and EEA-3135-HEM, with measurement of the rectal wall resected: vertical length in centimetres, horizontal length in centimetres, weight in grams and volume in cubic centimetres; afterwards histological study of the tissue thickness, and applied the ANOVA and SPSS 12 tests for the statistical analysis. Results: The results obtained by comparing the resections made with the CPH-34, the CPH-34HV and the EEA-3135-HEM in respect of vertical length, horizontal length, weight and volume, were found to have no significant differences; neither in the histological study of the tissue thickness in respect of characteristics and structure. Conclusion: The effect of mechanical technology in the treatment of anorectocele with transanal repair of rectocele and full rectal mucosectomy with one circular stapler procedure using the CPH-34, the CPH-34HV and the EEA-3135-HEM, does not show any difference. Leaving the application of each to the operator competencies.


RESUMO Objetivo: TRREMS (Transanal Repair of Rectocele and full rectal Mucosectomy with one circular Stapler, Reparo transanal de retocele e mucosectomia retal total com um grampo circular) é um procedimento que visa o tratamento da Síndrome da Defecação Obstrutiva pelo Dr. Francisco Sergio Regadas em 2005. Comparamos o uso de diversos instrumentos e o efeito mecânico da tecnologia no tratamento da anorretocele. Pacientes e métodos: Pacientes do gênero feminino com queixa de sensação de evacuação incompleta (SEI), com idades entre 40 e 55 anos. A avaliação foi efetuada com o protocolo de função de evacuação: tempo de trânsito colônico, radiologia do cólon, ecodefecografia, manometria anorretal e teste psicológico. A técnica empregada foi TRREMS, com o uso de grampeadores CPH-34, CPH-34HV e EEA-3135-HEM, com medição da parede retal ressecada: comprimento vertical em centímetros, comprimento horizontal em centímetros, peso em gramas e volume em centímetros cúbicos; subsequentemente, foi realizado estudo histológico da espessura do tecido, com aplicação de ANOVA e do programa SPSS 12 para a análise estatística. Resultados: Observamos que os resultados obtidos com a comparação das ressecções realizadas com CPH-34, CPH-34HV e EEA-3135-HEM com relação ao comprimento vertical, comprimento horizontal, peso e volume, bem como os resultados do estudo histológico da espessura do tecido com relação às características e estrutura, não apresentavam diferenças significativas.

We report a case of one of the rare complications of constipation in an adolescent. The patient presented with acute abdominal symptoms and the radiographic images revealed a dilated twisted sigmoid colon. Rectal tube reduction was successful and subsequently, the patient underwent sigmoidectomy and uneventful recovery

Resumen:Antecedentes:el estreñimiento crónico en la población pediátrica es una patología frecuente que puede afectar la calidad de vida del paciente y el entorno familiar. Se presenta la experiencia inicial de la Clínica de Manejo Intestinal, en la que se incluyen pacientes con patologías médicas y quirúrgicas de difícil manejo.Metodología:se revisaron los expedientes clínicos de la Clínica de Manejo Intestinal del Hospital "Dr. Maximiliano Peralta Jiménez", incluyendo todos los pacientes valorados desde junio de 2012 hasta junio de 2015. Se tomaron como pacientes activos, aquellos que se encuentran en control en la clínica, y como resueltos aquellos pacientes que fueron manejados inicialmente y luego referidos a la consulta externa de pediatría o cirugía.Resultados:se obtuvieron 77 pacientes, 25 activos (35%) y 52 resueltos (65%), que fueron referidos de las consultas de Pediatría y Cirugía Pediátrica, por estreñimiento de difícil manejo. De los pacientes activos, los diagnósticos son: 16 pacientes (64%) estreñimiento funcional, 6 pacientes (24%) enfermedad de Hirschsprung, 1 paciente (4%) con Síndrome de Mowat Wilson, 1 paciente con espina bífida, 1 paciente con ptosis del colon transverso. El motivo de consulta de los pacientes activos fue estreñimiento crónico en 20 (80%), enfermedad de Hirschsprung en 3 (12%) y encopresis en 2 pacientes (8%); los síntomas asociados de este grupo de pacientes fueron: dolor abdominal en 10 (40%), vómito en 3 (12%) y manchado en 19 (76%). De los 52 pacientes resueltos, 32 son hombres (61,5 %) y 20, mujeres. 44 pacientes (84,6%) tienen diagnóstico de estreñimiento funcional, 6 (11,5%) de enfermedad de Hirschsprung, 1 (1,9%) de ano imperforado con fístula perineal, y 1 con estenosis anal. El motivo de consulta fue estreñimiento crónico en 47 pacientes (90%), estenosis anal en 1 (1,9%), enfermedad de Hirschsprung en 1 (1,9%), fecalomas en 1 (1,9%), malformación anorrectal en 1 (1,9%) y enfermedad pélvica inflamatoria también en 1 paciente (1,9%).Conclusiones:los resultados obtenidos permiten identificar las causas más frecuentes de referencia de los pacientes con estreñimiento referidos a la Clínica de Manejo Intestinal del Hospital "Dr. Maximiliano Peralta Jiménez". De todos los pacientes referidos por estreñimiento de difícil manejo, se detectaron 15 (19%) de resolución quirúrgica.


Abstract:Background:Chronic constipation in the pediatric population is a common pathology that can affect the quality of life of the patient and the family. The initial experience at the Bowel Management Clinic is presented; patients with medical and surgical pathologies are included.Methodology:the clinical files from the Bowel Management Clinic at the Hospital "Dr. Maximiliano Peralta Jiménez" were reviewed, including all the patients evaluated between June 2012 to June 2015. The patients considered as active, are those who are presently in control at the clinic and patients considered as solved those patients who were evaluated initially and then referred to the pediatric outpatient clinic or surgery.Results:77 patients were included, 25 were active patients (35%) and 52 solved (65%), that were referred from the pediatric and pediatric surgery outpatients clinic for difficult management constipation. In the active patients the diagnoses are: 16 patients (64%) functional constipation, 6 patients (24%) Hirschsprung's disease, 1 patient (4%) with Mowat Wilson's Syndrome, 1 patient with spina bifida and 1 patient with ptosis of the transverse colon. The motive for consultation of the active patients was a chronic constipation in 20 patients (80%), Hirschsprung's disease in 3 (12%) and encopresis in 2 patients (8%); the associated symptoms of this group of patients were: abdominalpain in 10 patients (40%), vomit in 3 patients (12%) and soiling in 19 patients (76%) From the 52 solved patients, 32 are men (61.5%) and 20 women. 44 patients (84.6%) have diagnosis of functional constipation, 6 (11.5%) Hirschsprung's disease, 1 patient (1.9%) imperforated anus with perineal fistula and 1 patient with anal stenosis. The motive for consultation was chronic constipation in 47 patients (90%), anal stenosis in 1 (1.9%), anorectal malformation in 1 (1.9%) and inflammatory pelvic disease also in 1 patient (1.9%).Conclusions:The obtained results allow us to identify the most frequent causes of referral patients with chronic constipation to the Bowel Management Clinic at the Hospital "Dr. Maximiliano Peralta Jiménez". From all the patients referred for constipation of difficult management 15 (19 %) were detected for surgical resolution.

ABSTRACT

Abstract Chronic constipation in children and adolescents is relatively common and a reason for consultation with pediatricians and proctologists. Most cases respond to medical treatment. Advanced cases of megacolon and megarectum can be treated surgically by Duhamel technique. This case report describes a 15-year-old patient with chronic intestinal constipation refractory to clinical treatment associated with megacolon and megarectum, which was surgically treated.


Resumo A constipação intestinal crônica em crianças e adolescentes é relativamente comum e motivo de consultas a pediatras e coloproctologistas. A maioria dos casos responde ao tratamento clínico. Casos avançados de megacolon e megarreto podem ser tratados cirurgicamente através da cirurgia de Duhamel. Este relato de caso descreve um paciente de 15 anos de idade com quadro de constipação intestinal crônica refratária ao tratamento clínico associada a megacolon e megarreto, o qual foi tratado cirurgicamente.

ABSTRACT Background - Several scoring was developed for evaluation of children with fecal retention using plain radiograph. There are controversies about specificity and sensitivity of these scoring system. Objectives - The aim of this study was to evaluate Barr, Blethyn, and Leech score in evaluation of fecal load in plain radiograph. Methods - This case control study was conducted on children aged 2-14 years old with abdominal pain who visited Abuzar children's Hospital of Ahvaz University of Medical Sciences. This study was conducted in fall season. Children with history of previous abdominal surgery, any systemic illness including sickle cell anemia were excluded. Children with constipation were placed in case group. Subjects without constipation were placed in control group. Subjects without exclusion criteria were examined by physician who is blind to aim of the study. Careful history and physical examination was done. Demographic features, history of gastrointestinal problem, duration of abdominal pain, defecation habit, stool consistency (loose, hard), and results of physical examination were recorded. Rome III criteria was used for definition of constipation. Abdominal x-ray was ordered for each patients. Abdominal radiography was reviewed by radiologist. Barr, Leach, and Blethyn scores were calculated for each case. Results - In this study 102 children with functional constipation and 102 children without constipation as a control were included. Mean ±SD for case and control group was 68.39±34.88 and 69.46±32.60 (P=0.82).Leech score (mean ±SD) was 11.05±2.177 and 5.67±3.228 for case and control group respectively (P<0.0001). Barr score (mean ±SD) was 14.86±3.54 and 7.16±5.59 for case and control group respectively (P=<0.0001). Blethyn (mean ±SD) score was 1.97±0.667 and 1.04±0.900 for case and control group respectively (P=0.000). Sensitivity and specificity of Barr score was 83% and 79% respectively. Sensitivity and specificity of Leech score was 92% and 80% respectively. Sensitivity and specificity of Blethyn score was 79% and 92% respectively. Conclusion - Barr, Blethyn and Leech scores were significantly higher in children with abdominal pain and constipation in contrast to children with abdominal pain and without constipation. Sensitivity of Leech score was more than Barr and Blethyn scoring systems. Specificity of Blethyn score was more than Barr and Leech score.


RESUMO Contexto - Diversos métodos de pontuação utilizando a radiografia simples foram desenvolvidos para a avaliação de retenção fecal em crianças. Há controvérsias sobre a especificidade e sensibilidade destes sistemas de pontuação. Objetivo - O objetivo deste estudo foi avaliar os sistemas de escore Barr, Blethyn e Leech na avaliação do conteúdo fecal pela radiografia simples. Métodos - Estudo de caso controle em crianças com idade entre 2-14 anos, com dor abdominal que consultaram o Abuzar children's Hospital of Ahvaz University of Medical Sciences, durante o outono. Crianças com história prévia de cirurgia abdominal, com qualquer doença sistêmica, incluindo anemia falciforme, foram excluídas. Crianças com constipação foram inseridas no estudo e crianças sem constipação inseridas no grupo controle. Pacientes sem critérios de exclusão foram examinados por médico que ignorava o objeto do estudo. Foram realizadas história clínica e exame físico cuidadosos. Foram registradas as características demográficas, a história do problema gastrointestinal, a duração da dor abdominal, os hábitos intestinais e evacuatórios, a consistência das fezes (duras, amolecidas), e os resultados do exame físico. Os Critérios de Roma III foram usados para a definição de constipação. Radiografia simples do abdômen foi ordenada para cada paciente. Foram calculados os escores de Baar, Leech e Blethyn para cada paciente. Resultados - Foram incluídas neste estudo 102 crianças com constipação funcional e 102 crianças sem constipação como controle. A idade média com desvio padrão para o grupo paciente foi de 68.39±34.88 e de 69.46±32.60 para o grupo controle (P=0.82). O escore de Leech (média ±DP) foi de 11.05±2.1777 para pacientes e de 5.67±3.228 para o grupo controle (P<0.0001). A pontuação de Barr (média ± DP) foi 14.86±3.54 para pacientes e de 7.16±5.59 para o grupo controle (P=<0.0001). O escore Blethyn (média ±DP) foi de 1.97±0.667 para pacientes e de 1.04±0.900 para o grupo controle (P=0.000). A sensibilidade e especificidade para o escore de Baar foi de 83% e 79% respectivamente. Para o escore de Leech foi de 92% de sensibilidade e 80% de especificidade. O escore de Blethyn resultou em 79% de sensibilidade e especificidade de 92%. Conclusão - As pontuações de Baar, Blethyn e Leech foram significativamente maiores em crianças com dor abdominal e constipação em contraste com as crianças com dor abdominal e sem constipação. A sensibilidade de pontuação Leech foi maior do que os sistemas de pontuação Barr e Blethyn. Escore de Blethyn teve mais especificidade que as pontuações Barr e Leech.

ABSTRACT

Objective: To report a case of a preschool girl who developed acute urinary retention associated with constipation. Case description: A girl aged six years old presented a 24 h history of inability to urinate. She was went twice to the emergency room during this period. In the first admission, 12 h after the onset of the symptoms, she presented abdominal pain and acute urinary retention. After the drainage by urinary catheterization of 300 mL of clear urine, she presented relief of the symptoms and, as urinalysis had no change, the patient was discharged home. Twelve hours after the first visit, she returned to the emergency room complaining about the same symptoms. At physical examination, there was only a palpable and distended bladder up to the umbilicus with no other abnormalities. Again, a urinary catheterization was performed, which drained 450 mL of clear urine, with immediate relief of the symptoms. Urinalysis and urine culture had no abnormalities. During the anamnesis, the diagnosis of constipation was considered and a plain abdominal radiography was performed, which identified large amount of feces throughout the colon (fecal retention). An enema with a 12% glycerin solution was prescribed for three days. During follow-up, the child used laxatives and dietary modifications, this contributed to the resolution of the constipation. There were no other episodes of urinary retention after 6 months of follow-up. Comments: Acute urinary retention in children is a rare phenomenon and constipation should be considered as a cause.


Objetivo: Relatar um caso de criança que desenvolveu retenção urinária aguda associada à constipação intestinal. Descrição do caso: Menina, seis anos, havia 24 horas apresentara incapacidade de liberação de esfíncter vesical. Foi atendida por duas vezes em um serviço de emergência nesse período. Na primeira consulta, 12 horas após o início do quadro, apresentava dor abdominal e retenção urinária aguda e foi feita sondagem de alívio com saída de 300mL de urina clara. Houve alívio imediato dos sintomas e, como o exame de urina tipo 1 não apresentou alterações, a paciente recebeu alta. No segundo atendimento, 12 horas após a primeira consulta, apresentava as mesmas queixas. Ao exame físico, observou-se apenas bexiga palpável e distendida até a cicatriz umbilical, sem outras alterações. Nova sondagem vesical foi feita com saída de 450mL de urina clara, com alívio imediato dos sintomas. Nenhuma anormalidade foi observada no exame de urina tipo 1 e na urocultura. Durante a anamnese, foi levantada a hipótese diagnóstica de constipação intestinal. Foi feita radiografia simples de abdome, que identificou grande quantidade de fezes em todo o cólon (retenção fecal). Enema com solução glicerinada a 12% foi prescrito por três dias. Durante o seguimento a criança fez uso de laxativos e modificações na dieta que contribuíram para a resolução da constipação intestinal. Não houve repetição do quadro de retenção urinária aguda após seis meses de acompanhamento. Comentários: A retenção urinária aguda em crianças é um fenômeno raro e a constipação intestinal deve ser considerada como uma das causas.

Introducción: La cirugía del prolapso hemorroidal puede ser controversial, en particular cuando éste es circunferencial o en corona; diversos procedimientos han sido propuestos, algunos de ellos con complicaciones difíciles de solucionar. La técnica de Buie modificada puede resolver esta patología evitando las mismas. Objetivo: Evaluar prospectivamente los resultados obtenidos en la cirugía del prolapso hemorroidal mediante el empleo de la técnica de Buie modificada, sola o combinada con otra técnica. Diseño: Prospectivo observacional. Pacientes y Método: 37 pacientes consecutivos, operados entre agosto de 2007 y mayo de 2014, por prolapso hemorroidal con la técnica de Buie modificada, sola o combinada con otra técnica. Resultados: La edad promedio de la muestra fue 46,48; de los cuales el 62,16% fueron de sexo masculino; todos los pacientes eran portadores de un prolapso hemorroidal parcial o circunferencial o en corona. El sangrado ocasional fue el síntoma más frecuente (75,67%), seguido de la humedad perianal (67,57%). La técnica de Buie modificada se llevó a cabo solo en el 32,43% y combinado con la técnica de Ferguson en el 67,57%. El promedio de días de internación fue de 1,11. Cinco pacientes evidenciaron un sangrado escaso y de presentación irregular siendo la única complicación postoperatoria (13,5%). En el control a 30 días todos los enfermos tuvieron una adecuada cicatrización. No se constataron estenosis anal o ano húmedo por ectropión mucoso. Conclusiones: La técnica de Buie modificada sola o combinada resultó un procedimiento adecuado y efectivo para el tratamiento del prolapso hemorroidal con una estadía sanatorial corta, bajo costo y un índice de complicaciones postoperatorias más que aceptables.


Introduction: Prolapsed hemorrhoid surgery can be controversial, in particular when the surgery involves prolapsed hemorrhoids partial or in “crown”. Several procedures have been proposed, some of which have provided with complications difficult to solve. Buie’s modified technique can resolve this pathology, preventing these complications. Objective: To assess prospectively the acquired results obtained from prolapsed hemorrhoid surgery after Buie’s modified technique, alone or in combination with another technique. Design: Prospective observational. Patients and Method: 37 consecutive patients underwent surgery after Buie’s modified technique alone, or combined with another technique, between August 2007 to May, 2014. Results: The average mean age of the patients was 46,48; from which 62, 16% were males; all carried partial or in “crown” prolapsed hemorrhoids. Occasional bleeding was the most frequent symptom (75, 67%), followed by perianal moisture (67,57%). The Buie’s modified technique was performed alone in 32,43% and in combination with the Ferguson technique in 67, 57%. The average days of patients admission was 1,11. Five patients showed scanty and irregular bleeding, this being the only post-operative complication (13,5%). After a 30-day checkup, all patients presented reasonable scaring. No evidence was found for anal stenosis or moist anus by mucosal ectropion. Conclusions: The Buie’s modified technique, alone or combined, turned out to be the most effective and proper procedure for the treatment of prolapsed hemorrhoids; it is inexpensive, besides the patient is admitted to the hospital for a short period of time with a more acceptable index of post-operative complications.

Introducción: Las hemorroides son una patología muy frecuente. La mayor parte de los pacientes son tratados con modificaciones en la dieta, aquellos que no responden al tratamiento son pasibles de tratamientos ambulatorios y los casos más avanzados quirúrgicos. Objetivos: Estudiar la evolución alejada de los pacientes con patología hemorroidal severa. Diseño: Estudio retrospectivo con encuesta telefónica. Material y Métodos: La serie estudiada son 304 pacientes operados entre 1980 y 2012, correspondiendo 124 a casos con más de 50% de la circunferencia comprometida por trombosis, gangrena o casos de importante prolapso, a quienes se efectuó la exéresis de las zonas afectadas y la reconstrucción del canal anal utilizando colgajos cutáneos, y 180 fueron tratados de tres o más paquetes con técnica de Ferguson. Resultados: En el período considerado consultaron por hemorroides 1832 pacientes, de los cuales a 180 (9,8%) se utilizó la técnica de Ferguson para tres o más paquetes y 124 (6,8%) fueron casos complejos. Presentaron complicaciones post operatorias el 13,71% de las complejas y el 6,11% de tres paquetes o más (n s). Por medio de la encuesta encontramos en los casos denominados complejos un 93% de asintomáticos y en los de más de 3 Paquetes un 81%, resultando esta diferencia significativa. Los síntomas más frecuentes, sangrado y dolor, fueron muy esporádicos en el 71,4% vinculado a constipación o transgresiones alimentarias manifestando el 93,5% sentirse mejor que antes de operados. Conclusiones: Tanto los resultados inmediatos como el seguimiento alejado en la serie presentada han sido muy satisfactorios.


Introduction: Hemorrhoidal disease is a very frequent pathology. Most hemorrhoids are medically treated with diet changes. Those who do not respond satisfactorily can receive office-based treatments and the most advanced cases require surgical intervention. Aims/objectives: Study of the long-term evolution of patients with severe hemorrhoidal pathology. Design: Retrospective study with telephonic survey. Material and Methods: The studied series comprises 304 patients operated on between 1980 and 2012, corresponding 124 cases to patients with more than 50% of the anal circumference compromised by thrombosis, gangrene or cases of significant prolapse. These cases underwent exeresis of the affected areas and reconstruction of the anal canal utilizing cutaneous flaps, and 180 cases, which presented more than three bundles, were treated with Ferguson´s technique. Results: In the referred period, 1832 patients consulted about hemorrhoids. Ferguson´s technique was used on 180 patients (9,8%) and 124 patients (6,8%) corresponded to complex cases. Post-surgical complications were seen in 13, 71% of the complex cases and in 6.11% of the cases with three or more hemorrhoidal bundles (n s). The survey showed that 93% of the complex cases were asymptomatic, in comparison to 81% of those with three of more hemorrhoidal bundles, resulting in a significant difference. The most frequent symptoms: bleeding and pain, were very sporadic in 71, 4% corresponding to constipation or dietary transgressions; 93, 5% stated feeling better than before being operated on. Conclusions: The immediate results as well as the long-term follow up of the presented series have both been very satisfactory.

ABSTRACT

OBJETIVO: Descrever a avaliação nutricional de crianças com paralisia cerebral, verificando a concordância de curvas de crescimento específicas para paralisia cerebral com curvas gerais, além de avaliar a presença das manifestações digestórias associadas a agravos nutricionais. MÉTODOS: Corte transversal de 187 indivíduos com paralisia cerebral, avaliando-se dados antopométricos em curvas habitualmente utilizadas em pediatria e em curvas específicas para paralisia cerebral, além da descrição da presença de disfagia, constipação intestinal e infecções respiratórias de repetição. RESULTADOS: Dos indivíduos, 58% eram masculinos; média de idade de 5,6 anos ± 3,5 anos. Dados antopométricos de peso abaixo do percentil 10 ocorreu em 10% da amostra, considerando a escala para paralisia cerebral, contra 51% na referência do Center of Disease Control (p < 0,01; Kappa 0,19). A maioria dos indivíduos comprometidos com disfagia, infecções respiratórias de repetição e constipação intestinal encontrava-se abaixo do percentil 50, com porcentagens respectivas de 67%, 75% e 72%. CONCLUSÃO: As referências habitualmente utilizadas em pediatria tendem a superestimar a desnutrição em indivíduos com paralisia cerebral, e sua concordância com referências específicas para paralisia cerebral é baixa. As manifestações digestórias foram encontradas principalmente naqueles indivíduos cujas medidas antropométricas encontram-se abaixo do percentil 50.


OBJECTIVE: To describe the nutritional assessment of children with cerebral palsy, verifying the correlation of growth curves specific for cerebral palsy with general curves, in addition to assessing the presence of digestive manifestations associated with nutritional problems. METHODS: This was a cross-sectional study of 187 individuals with cerebral palsy, evaluating anthropometric data in curves commonly used in pediatrics and specific curves for cerebral palsy, in addition to the description of presence of dysphagia, constipation, and respiratory infections. RESULTS: 58% of patients were males, with a mean age of 5.6±3.5 years. Anthropometric data of weight below the 10th percentile occurred in 10% of the sample considering the cerebral palsy scale, versus 51% when considering the reference from the Centers for Disease Control and Prevention (p < 0.01; Kappa 0.19). The weight of most individuals with dysphagia, recurrent respiratory infections, and constipation was below the 50th percentile, with respective percentages of 67%, 75%, and 72%. CONCLUSION: The references commonly used in pediatrics tend to overestimate malnutrition in individuals with cerebral palsy, and their correlation with specific references for cerebral palsy is low. Digestive manifestations were mainly found in those individuals whose anthropometric measurements were below the 50th percentile.

JUSTIFICATIVA E OBJETIVOS: Estudos recentes têm identificado a constipação intestinal (CI) como fator prognóstico independente na evolução de pacientes críticos e mostrado que seu tratamento pode resultar em melhor prognóstico. O objetivo deste estudo foi avaliar a incidência de CI em pacientes com nutrição enteral internados em unidade de terapia intensiva (UTI) e sua associação com o tempo de ventilação mecânica (VM), de internação e a mortalidade. MÉTODO: Foram incluídos prospectivamente 26 pacientes de ambos os sexos, acima de 18 anos, com uso de dieta enteral de forma exclusiva por no mínimo três dias durante a internação na UTI, de novembro de 2011 a janeiro de 2012. Foram registrados dados demográficos, diagnósticos, dias em VM, duração da terapia de nutrição enteral, hábito intestinal e desfecho clínico. RESULTADOS: A idade média dos pacientes foi 60,5 ± 18,7anos, sendo que 50% eram homens. O tempo de VM foi de 9,0± 7,3 dias e a mediana do tempo de internação na UTI foi de 13,0 (7,0-18,0) dias. Os pacientes constipados (n = 13; 50%) demoraram mais tempo para atingir a meta nutricional prescrita. Além disso, apresentaram tempo de VM duas vezes maior que os sem constipação. Em análise de regressão linear para predição de tempo de VM, a constipação foi preditora de maior tempo de VM, mesmo após ajuste por gênero e gravidade (p = 0,043). No entanto, a CI não esteve associada ao tempo de internação na UTI ou ao óbito. CONCLUSÃO: A presença de CI em pacientes críticos é preditora de maior tempo de VM. Portanto, a criação e implantação de protocolos para o seu tratamento na UTI é de fundamental importância.


BACKGROUND AND OBJECTIVES: Recent studies have demonstrated that intestinal constipation (IC) is an independent prognostic factor in critically ill patient's evolution. In addition, constipation treatment can result in better outcome. The purpose of this study is to evaluate the incidence of IC in intensive care unit (ICU) patients receiving enteral nutrition, and its association with duration of mechanical ventilation (MV) and length of stay, and mortality in ICU. METHOD: We prospectively evaluated 26 patients of both genders, older than 18 years, exclusively receiving enteral nutrition for at least three days who were admitted to an ICU from November/2011 to January/2012. Demographic data, diagnosis, duration of MV and enteral nutrition, bowel habits, and outcomes were registered. RESULTS: Patients' average age was 60.5 ± 18.7 years old, and 50% were male. Duration of MV was 9.0 ± 7.3 days and median length of ICU stay was 13.0 (7.0-18.0) days. Patients with constipation (n = 13, 50%) took longer time to meet nutritional requirements. In addition, duration of MV was twofold higher in IC patients compared to non-IC patients. In linear regression analysis for prediction of MV time, IC was a predictor of longer duration of MV, even after adjustment for gender and illness severity (p = 0.043). However, IC was not associated with length of stay in ICU or mortality. CONCLUSION: The presence of constipation in critically ill patients predicts longer duration of MV. Therefore, creation and implementation of clinical protocols for IC treatment is of fundamental importance.

Relatar dois casos de doença celíaca (DC) com manifestação de constipação. DESCRIÇÃO DO CASO: Dois pacientes do sexo feminino, com 18 e 30 meses de idade, respectivamente, apresentando história de constipação crônica refratária ao tratamento. Como apresentavam concomitantemente baixo ganho ponderal e estatural, foi realizada investigação da função digestiva-absortiva, que resultou positiva para o anticorpo IgA antitransglutaminase tecidual. O diagnóstico de DC foi confirmado por biópsia de intestino delgado que revelou atrofia vilositária moderada/intensa e infiltrado linfocítico intraepitelial. Um mês após o início do tratamento com dieta isenta de glúten, ambas as pacientes passaram a apresentar fezes pastosas diariamente. COMENTÁRIOS: A DC pode se apresentar nas formas clássica, assintomática e atípica, em que manifestações isoladas como constipação podem retardar o diagnóstico.


To report two cases of patients with celiac disease (CD) whose main complaint was chronic constipation. CASE DESCRIPTION: Two girls, aged 18 and 30 months, had chronic constipation refractory to standard treatment. Both patients concomitantly evidenced low weight gain and short stature. The investigation of the digestive-absorptive function was positive for IgA antibodies against tissue transglutaminase. The diagnosis of CD was confirmed by a small bowel biopsy that showed moderate/severe villous atrophy and increased intraepithelial lymphocytic infiltration. One month after starting the dietary treatment with a gluten-free diet, both patients recovered from constipation, with the passage of soft stools daily. COMMENTS: CD may be presented in the classical, asymptomatic or atypical forms. In the latter form, isolated manifestations, surch as constipation, delay the diagnosis of the disease.


Relatar dos casos de enfermedad celíaca (EC) con manifestación de constipación. DESCRIPCIÓN DEL CASO: Dos pacientes del sexo femenino, con 18 y 30 meses de edad, respectivamente, presentando historia de constipación crónica refractaria al tratamiento. Como presentaban concomitantemente baja ganancia de peso ponderal y estatural, se realizó investigación de la función digestiva-absortiva, que resultó positiva para el anticuerpo IgA antitransglutaminasa tejidual. El diagnóstico de EC fue confirmado por biopsia de intestino delgado que reveló atrofia vellositaria moderada/intensa e infiltrado linfocítico intraepitelial. Un mes después del inicio del tratamiento con dieta exenta de gluten, ambas pacientes pasaron a presentar heces pastosas diariamente. COMENTARIOS: La EC puede presentarse en la forma clásica, asintomática y atípica, en que manifestaciones aisladas como constipación pueden retardar el diagnóstico.
